- Unlimited X-rays (3D cone beam excluded)
- 20% off most treatments and services
+Exclusions
- Our membership is NOT a dental insurance plan and can only be used at Smile Quest Dental.
- The membership is non-transferable.
- No discount is given if third-party financing is used.
- Discounts under your membership do not apply to any treatment prior to joining.
- The 12-month membership fee is due in full upon joining when applying a reduced fee schedule toward your treatment.
- Payments are due when services are rendered.
- It is the sole responsibility of the member to maximize benefits by arranging all appropriate appointments within the 12-month membership period.
- If benefits are not used, the member will not be entitled to a refund.
- Renewal payment is due at the beginning of the same month of each year.
- The annual renewal month of your membership is the month you originally joined, regardless of when payment is received.
- Fees are based on the usual and customary fee at the time of service.
