Our membership is NOT a dental insurance plan, and can only be used here at Smile Quest Dental.
The membership is non-transferable.
NO discount given if 3rd party financing is used.
Discounts under your membership do not apply to any treatment prior to joining.
12-Month membership fee is due in full upon joining when applying reduced fee schedule toward your treatment.
Payments are due when services are rendered.
It is the sole responsibility of the member to maximize benefits by arranging all appropriate appointments within the 12-month membership period.
If benefits are not used, the member will not be entitled to a refund.
Renewal payment is due at the beginning of the same month each year.
The annual renewal month of your membership is the month you originally joined, regardless of when payment is received.
